This service will be provided to all individuals with developmental disabilities under Medicaid. Our support coordinator will work with the client and their family members to assess the client’s condition, complete the PCPT or the person-centered planning tool, and develop ISP or individualized service plan.
Our support coordinators will help clients with, but are not limited to, the following:
- Enhance their knowledge, experience, and connections with the entire community and other support systems.
- Assess and evaluate the clients’ skills and self-sufficiency to better understand their capability and design appropriate support solutions that will help them meet their desired outcome from the service.
- Enable clients to know various support options they can consider to instill the right option in their service plan.
- Assist clients in resolving problems, allowing them to learn problem-solving skills and resiliency, along the way.
- Allow clients to maintain, manage, and improve their relationship with the community.
